pronounced. This professional learning session equips 
educators with practical, research-based strategies to 
support struggling math learners in grades 3–5. 
Participants will explore how to strengthen foundational 
skills while promoting conceptual understanding, 
problem-solving, and mathematical discourse. Educators 
will learn how to use formative assessment to pinpoint 
misconceptions and design targeted interventions to meet 
diverse learning needs.

Crafting Impactful 
Transition Plans 
for Students with

The Individuals with Disabilities Education Act (IDEA) 
defines transition services as a “coordinated set of 
activities” that are individualized and results-oriented to 
support students with disabilities in moving successfully to 
post-secondary environments. Successful transitions, 
regardless of a student’s age, require thoughtful and 
purposeful planning. This course will equip participants 
with the knowledge and skills necessary to write 
comprehensive annual transition plans that meet IDEA 
requirements and effectively prepare students for life 
beyond school. Participants will learn how to incorporate 
transition assessments, design a course of study, develop 
measurable goals, and outline appropriate transition 
activities and services. By the end of this course, 
educators will be able to create meaningful transition 
plans that foster student growth, independence, and 
success in post-secondary settings, ensuring that each 
student’s transition journey is well-supported and 
impactful.
